Forms can either be filled out online or downloaded as a pdf for completion offline. For a pdf document that has been saved on your computer, you may either enter information directly onto the pdf or print out to fill in by hand. Completed forms may be emailed to info@cellbankaustralia.com (if information is entered directly onto the pdf) or faxed to +61 2 9678 2120.

Details of the services provided can be found within the Services pages of the website. Pricing for Mycoplasma and Identification testing is per sample or per cell line; a discount applies for batches submitted at one time. For culture and storage services, pricing may vary with the cell line or number of vials to be sent, so you will need to contact us for a quote.

Account Application - click here

New customers using our services for the first time will need to fill in a Client Account application form. This gives us your contact details and also allows customers who wish to pay by purchase order to set this up prior to sending samples.

Mycoplasma Request for Analysis - click here

This form should be filled out before you send your Mycoplasma testing samples to us. We ask you to supply some basic details about each sample and fill in a biohazard risk assessment. Instructions are also included for how to prepare your sample and where to send it to.

This form should be filled out before you send your identification testing samples to us. The deposit query is a form for researchers who are considering depositing their cell line for distribution by the repository. It captures the basic information required for us to give feedback on whether this is appropriate e.g. whether we can contain any risks appropriately.

After a researcher completes the Deposit Query, we will ask you to fill out this form so that we have more information to create a catalogue entry for the cell line.
