Notification of Intent to Submit a Patent Deposit
Before you send your Patent Deposit you must complete this form and return a signed copy of this form to CellBank Australia. This captures the basic information we need to make a decision on whether the cell line is suitable for submission e.g. a safety assessment to determine its risk.
Depositor Information
The deposit is made in accordance with the terms of the Budapest Treaty 1977. I agree to abide by the conditions and regulations regarding the deposit of cell and hybridoma lines with CellBank Australia.

Safety Assessment
For additional information and detailed lists of organisms in each grouping, see AS/NZ 2243.3 2002 – Safety in Laboratories Part 3: Microbiological Aspects and Containment Facilities. Risk Groups 3 and 4 cannot be accepted by CellBank Australia. Cell lines derived from human clinical material with no known infectious disease are usually classified as risk group 2.
The definition of a genetically modified organism (GMO) can be found in the Gene Technology Act 2000 and on the website of the Office of the Gene Technology Regulator (http://www.ogtr.gov.au/).

| What risk group does the cell belong to? |
Low individual and community risk, an organism that is unlikely to cause human, plant or animal disease. |
Moderate individual risk and limited community risk. A pathogen that can cause human, plant or animal disease, but is unlikely to be a serious hazard to laboratory workers, the community, livestock or the environment. Laboratory exposures may cause infection, but effective treatment and preventive measures are available, and the risk of spread is limited. |
High individual risk and limited community risk. A pathogen that usually causes serious human or animal disease and may present a serious hazard to laboratory workers. It could present a risk if spread in the community or the environment, but there are usually effective preventive measures or treatment available. |
High individual and community risk. A pathogen that usually produces life-threatening human or animal disease: represents a serious hazard to laboratory workers and is readily transmissible from one individual to another. Effective treatment and preventive measures are not usually available. |

Possible infectious agents include viral vectors expressing SV40 virus, adenovirus, or cytomegalovirus. |
